FOXO4-DRI is a D-retro-inverso isoform peptide designed to disrupt the FOXO4-p53 interaction in senescent cells; normally, FOXO4 sequesters p53-pSer15 in the nucleus of senescent cells to prevent apoptosis, and FOXO4-DRI competes with FOXO4 for p53 binding, selectively causing nuclear exclusion of p53 and intrinsic (mitochondria-directed) apoptosis specifically in senescent cells (https://pmc.ncbi.nlm.nih.gov/articles/PMC5556182/). The original 2017 Cell study by Baar et al. demonstrated selective killing of senescent fibroblasts and chemotherapy-induced senescent cells in vivo, restoring tissue homeostasis with tolerable toxicity in aged XpdTTD/TTD mice (https://pmc.ncbi.nlm.nih.gov/articles/PMC5556182/). More recent research (2025) in keloid models shows FOXO4-DRI selectively induces apoptosis in senescent fibroblasts at lower IC50 values than non-senescent controls (34 μM vs. 94 μM) via p53-pS15 nuclear translocation (https://www.nature.com/articles/s42003-025-07738-0).
