B7-33 is a synthetic single-chain analogue of the B-chain of human relaxin-2 (H2 relaxin), designed to bind the relaxin family peptide receptor 1 (RXFP1) with functional selectivity. Unlike full relaxin-2, B7-33 preferentially activates the pERK1/2 signaling pathway over the canonical cAMP pathway in cells endogenously expressing RXFP1, an approach termed 'biased agonism' (https://pubs.rsc.org/en/content/articlelanding/2016/sc/c5sc04754d). In a murine myocardial infarction model, B7-33 significantly reduced infarct size (22% vs 45% in vehicle), preserved fractional shortening at 7 days post-MI, and attenuated endoplasmic reticulum stress markers including GRP78 in primary cardiomyocytes (https://pmc.ncbi.nlm.nih.gov/articles/PMC7428518/). Preclinical studies also indicate vasoprotective and antifibrotic properties consistent with relaxin-2 pharmacology, with lower synthesis costs owing to the absence of the disulfide A-chain.
